To find a Health Care Network provider:

1. Go to 
2. Click on 鈥淔ind A Provider鈥� (Located in the top right corner of home page)
3. Choose "Careworks TX HCN" from menu 
4. Choose 鈥淎ddress Search鈥�
5. Enter TX for state and enter the zip code
6. Enter distance in miles 

For Provider Type, choose 鈥淭reating Providers鈥� then click on "Find Providers" button at the bottom.
