With every document you submit, please make sure it is complete and signed. This relates
primarily to your Mentor Agreement, lab rotation reports and lab evaluations (CMB
only), Committee composition forms, Committee meeting reports and Program of Studies
(BCHS only).鈥� However, there could be other documents, like petitions or other educational
related items.
Submission of all documents is the responsibility of the student (not your PI/advisor
or dissertation committee members). Complete the document as necessary (i.e. collect
signatures if required, provide summaries, etc.) and submit them to me in the forms
via the not emails. Multiple forms can be submitted/uploaded at one time or individually
uploaded as required via the link.
